What is CHN Colorado Health Partnerships, LLC dba Colorado Health Networks (CHN) is comprised of providers, eight Community Mental Health Centers and ValueOptions, the leading public sector managed behavioral healthcare organization in the country. This Partnership includes three provider owned LLCs: Pikes Peak Medicaid LLC, SyCare LLC and West Slope Casa LLC. These three LLCs encompass eight community mental health centers with responsibility for community mental health services is 43 counties. The eight community mental-health centers that make up the two partnerships include: Colorado West Mental Health Center - Glenwood Springs http://www.cwrmhc.org Midwestern Colorado Mental Health Center – Montrose http://www.midwestmhc.org Pikes Peak Mental Health Center - Colorado Springs http://www.ppmhc.org San Luis Valley Mental Health Center - Alamosa http://www.slvmhc.org Southeast Mental Health Services- La Junta http://www.semhs.org Southwest Colorado Mental Health Center – Durango http://www.swcmhc.org Spanish Peaks Mental Health Center – Pueblo http://www.spmhc.org West Central Mental Health Center - Canon City http://www.wcmhc.org CHN believes individuals and families can and do recover from mental illness and devotes significant resources to helping people overcome the challenges of serious and persistent mental illness.
